I want to add checkbox and dropdown to my existing customized screen like in MIGO

Hi,

I want to add one check box and drop down to my existing customized screen like in MIGO.Here i am attaching my Customized screen and MIGO screen.In MIGO screen left side one check box and dropdown with default value collective slip is there,like that i want to develop in my customized screen.Please help me.Thanks
